E-script the online AUTOMATED DVA RAP SYSTEM                                       

E-script online creates Automated DVA RAP forms. This allows you to select approved product from the online catalogue. The system then produces a RAP form using the specified products and client information provided, which you can then print, sign and fax directly to Active Medical FAX 07 3899 8825. Your details remain on the system and are automatically entered on each RAP form you complete.
